Care Coordination – Registered Nurse

Shift: 8:00 AM – 4:30 PM

Experience:

• 24 months of role experience is required.

• Travel experience is required.

• Experience with Cerner is required.

Requirements:

• Candidates must have a California license in hand at the time of application.

• This role may require floating to additional units and locations

• Candidates must live at least 50 miles away from this facility in order to be considered a traveler when applying for this role.

• COVID vaccination required for onboarding. Religious and medical declinations accepted.

• COVID booster required for onboarding. Religious and medical declinations accepted.

Guaranteed Hours 40

Inpatient only. All inpatients requiring utilization review & discharge planning

Must have strong acute care hospital experience. At least 3 year of acute care hospital experience and case management

Must have utilization review & discharge planning experience. Nurse will need to be able to do UR as well as oversee DC planning.
